Update:
Found the part number 54315-SW5-982. No longer made and no replacement part number. So I am trying to find out what years are interchangeable (some forums say 1996-1998, some are saying 1998-2003). Likely will have to contact a company to manufacture one for me.. unless I get lucky on eBay or a junkyard.
